[Community_garden] Community Gardens outside the Twin Cities area (Greater MN)

Does anyone have information about community gardens outside the Twin
Cities area?  
 
In conjunction with the GardenWorks program of the Green Institute,
Master Gardeners are trying to gather information to compile a resource
list of community gardens throughout Minnesota.  There has been quite a
bit of work done to develop a resource list for the Twin City
metropolitan area but this information does not appear to be captured
(or at least not readily accessible in a centralized location) for the
greater MN area..
 
The kind of information that we are looking to collect would include: 

*	Garden Name
*	Type of community garden (rental plots, beautification, food
production, etc) 
*	Location 
*	Contact information 
*	Community/population served and who is eligible to participate
in the garden 
*	Any other information that you think would be helpful to know
about the garden
